PHYSICAL
AND IMMUNIZATION REQUIREMENTS
All
children must have a physical prior to starting in the program. A physician's
report must be filled out, signed, and dated by a physician. The form must be
updated yearly by your
physician. Complete dates for all immunizations are
required on the forms. Annual lead screenings are required by the State, starting
at 9 months of age. A child's health care summary must be completed at the time
of enrollment and parents must notify the Center of any changes. |
